| Page updated 25/08/2008 Registering with a GP doctor in this general practice in Lichfield is easy and you no longer need your current doctor's permission to change. We are a Personal Medical Services practice so registration is with the Minster Practice not an individual doctor. However, your medical card will bear the name of one of the partners until the national change in medical cards takes place. Our practice area is the boundaries of the City of Lichfield, within which you must live to be eligible for registration. The wider area of Lichfield District outside the City boundaries is NOT covered. The easiest way to register is to complete the details on your current medical card, sign it and send it to us. We will arrange the change of registration with the local health authority (PCT) who will obtain your records from your current doctor and send them to us. If you cannot find your medical card, we have copies of the
official Family doctor services registration form GMS1. The information required
on the form is: If you are from abroad, please give your first UK address where registered with a GP, the date you first came to live in the UK, and, if previously resident in the UK, the date of leaving. If you are returning from the Armed Forces, your address before enlisting, enlistment date and Service or Personnel number. You can sign on behalf of each of your family members. If you email the above information to Dr Causer MIKE (dot) CAUSER (at sign) NHS (dot) NET , we will complete the form(s) for you and send them to you for signature. Alternatively, you may be able to download the form from the NHS site at http://www.nhs.uk/england/forms/GMS1.pdf If so, complete the form, sign it and hand it in or send by post. Visitors to your household can be seen as temporary residents unless they are with you for 3 months or more in which case they are supposed to formally register as above. The practice is approved to provide contraceptive services to patients registered with any GP for all other medical services. Drs Nto and King provide child health surveillance services to all practice children aged under 5. This is supplemented by services from the Consultant in Child Health based at the Health Centre, Dr Mary Plant. A full range of private services is available including private consultations and medical examinations. Registration is not required for these services. Credit cards and cheques above guarantee card limit are not accepted. Most transactions are cash based if an account has not been opened.
