The Minster Practice, Lichfield

We use a digital camera for clinical photography (Fuji FinePix S602 Pro Zoom as used by the medical photography department @ Queen's Hospital Burton). This camera is used when a permanent record is required for follow up purposes, particularly of skin problems, or for sending an image to a specialist to assist in diagnosis, assessment or treatment.
You will be asked to sign a consent form to agree to the photograph being taken. The consent form includes details about how the image will be handled.
The image will never be used for public display or in any professional publication even in an anonymous form.
